Mysql database and Mysql user can be created by visiting "Mysql Database" section of cpanel control panel. Please make sure you also add mysql user to mysql database by visiting "Add User To Database" in same section.
Database name and database user will always start with account username, for example if you created database db1 and your account username is cpuser then database name would be
cpuser_db1
